& Payment Policies
We are very happy to be the preferred network provider for many major insurance companies, such as WDS, Premera, Metlife, Aetna, Ameritas, Guardian, United Concordia, Cigna and DSHS . We will also be happy to file insurance claims for all other dental insurance companies to make your dental appointment easy. We will make sure that you understand your benefits fully, so that you may concentrate on your child’s oral health care. Please also know that your insurance is a contract between you and your insurance company. We file dental claims as courtesy to our patients.
We request that you notify our office with at least 2 business days notice for any canceled or rescheduled appointments. We fully understand that a child may become ill without much warning or you may have a last-minute emergency. After the third late notice cancellation, your account will be charged $75 per child, for check-up appointment, or $100 per child for filling appointment. Any no shows will be an automatic $$75/ child for check-up appointment or $100/child for filling appointment.

We welcome questions about insurance, cost, proposed treatment options, or anything that comes to your mind regarding your child’s oral health care. Please remember that you are ultimately responsible for your account and balance. We have many families come to us because of our compassion for children and high quality of services. Dr. Chen and his staff firmly believe in doing things right the first time, rather than doing it over.
